Trading Days Off

Or, how to fill out a form 3189.

 

Here is a blank form.

 

 

You have to trade days off with someone in your T6 swing. 

 

When you fill out this form you only fill out the part that you are asking to change.  Nothing else. 

 

Put your name at the top.

 

The dates at the top for changing your schedule run from Saturday through Friday since that is how our work weeks run.

 

Under Scheduled Day(s) Off, you put your day off on the left, and the day off you are trading for on the right.

 

Sign, 8 digit employee number, date.  Obviously, Oliver has to sign and date his form.

 

Your pay location is 003 for MPO and 006 for Jayhawk.

 

Your union steward must also sign and date the form.  Report any issues to them.

 

Let’s say that you (Stan) want to take off May 5 for Cinco de Mayo in 2020, but your day off is May 7.  You check the schedule and see that Oliver is on your swing and has the 5th as his day off.  You ask, and he is willing to trade days off with you.  You both have to fill out form 3189.  Be nice and fill out his for him, since you are the one asking.
