How to fill out a 3971

For taking days off, this form must be filled out in triplicate.

 

1.  Your name, obviously – Last, First.

2.  Your eight digit employee number.

3.  The date you turned this form in.

4.  The number of hours you are requesting.  This will usually be in multiples of 8.

5.  Which office are you in, MPO or Jayhawk.  For CCAs, whichever office you are in when you fill this out.

6.  Usually not necessary.

7.  003 for MPO, 006 for Jayhawk.

9.  The beginning date of your request.  Unless you are asking for a partial day, the hour isn’t necessary.

10.  The last date of your request.  The last day you expect to be gone.

11.  What kind of leave are you taking?

12.  Usually left blank.  If you are on FMLA, write that here.  If you are turning back time off, write ‘turn back’ here.

13.  Sign and date.

 

Fill out in triplicate and turn in to your supervisor and get a signed and dated copy of when it was received.  You should receive a notice within 3 days of approval or disapproval.  Disapproval should come with a reason.  ‘Needs of the service’ is not a legitimate reason.

 

The latest day you can ask for a day off is the Monday prior to the week which you are considering.

 

Full weeks of vacation run from Monday through Sunday, except Christmas week.  Christmas week runs from December 25 through January 1 regardless of what days they fall on.  The dates for Christmas week will be on the leave request notice posted by October 15 for the next year.

 

The day or days between December 25 and the previous weekend, if any, are unique.  If there is only one day, it is bid as an incidental day.  If there is more than one day, it will be bid as a week.  The same is true for the day or days between January 1 and the next weekend, if any.
